Set apple tv output at 50hz 4k sdr which makes my lg oled tv do the conversion to hdr, this has sorted the picture. I doubt anybody really knows the reason for the problem. Upon pressing play the tv will wait to begin playback for the 2 seconds, and then start video perfectly in sync with the audio stream, and pressing pause the audio will continue for 2 seconds past the video then pause.
